This form involves a Deed of Donation by an Individual Donor of his/her personal properties to UPLB.

Proof of ownership of the personal properties should form part of the annex of the Deed of Donation (e.g. Official Receipt, Certificate of Registration, etc)

This form involves a Memorandum of Agreement (MOA) between UPLB and a Foreign Visiting Researcher that plans to conduct his/her research activity in any UPLB College or Unit. However, please note that before using this form, it is a pre-requisite that there must be a signed Memorandum of Understanding (MOU) between UPLB and the Foreign University where the visiting researcher originated.

The MOU should be facilitated first by the Office of the International Linkages (OIL) before proceeding with this MOA.

This form is a waiver-quitclaim-consent form that must be filled-up by Non-UPLB Interns planning to take his/her practicum training in UPLB, with the approval of his/her parents or guardians.

This contract is attached as Annex “A” in the Memorandum of Agreement on Non-UPLB Internship (LO Form 015 – 2023) and described in Item C.1 as the “Declaration of Consent with Waiver and Quitclaim”.

This form is a sample Memorandum of Agreement between UPLB and a higher education institution wherein the latter shall deploy their students to UPLB as part of their practicum/internship program.

A. Secretary Certificate

Often referred to as a corporate secretary’s certificate or secretary’s resolution, this is a legal document issued by the corporate secretary of a company or organization. This document outlines the specific decision, action, or resolution that has been approved by the company’s board of directors or another authorized governing body. For UPLB, this is to authorize the representative of the company to sign the Memorandum of Agreement for Scholarship.

B. Board Resolution

Also known as a corporate resolution or director’s resolution, this is a formal document that records the decisions, actions, or directives approved by the board of directors of the corporation. This document serves as an official record of the board’s decisions and is important for legal and administrative purposes.

This form provides the format of an agreement between UPLB and the scholarship provider. It outlines the terms and conditions under which the scholarship funds or benefits will be awarded to the recipient/UPLB student in compliance with existing university policies.

This form provides the format of an agreement between UPLB and the partner local government unit (LGU). It outlines the terms and conditions for the implementation of the UPLB Student Internship program.

This also includes Annex “A”, the format for the Sangguniang Panlalawigan/Bayan Resolution which will authorize the Mayor to sign in behalf of the LGU.

This is a waiver and quitclaim form that must be filled-up by the parents and/or guardians, and the UPLB students planning to take their practicum training in industries partnering with UPLB.

This contract is attached as Annex “C” in the following contracts:

1. Memorandum of Agreement on UPLB Internship with Academe-Industry Cooperation Agreements (AICA), LO Form 008 – 01 (2022); or
2. Memorandum of Agreement on UPLB Internship without AICA, LO Form 008 – 02 (2022).
